VA Application Checklist

 The following is a general list of items needed when applying for the Veterans Affairs Benefit Aid and Attendance. Please note that this a general list and not all on this list may apply to your specific case. Additionally, this list may not be all inclusive. For the VA Aid and Attendance Benefit please provide the following:

  • Veteran’s original or certified copy of discharge papers.
  • Veteran and Spouse’s social security cards.
  • Veteran and Spouse’s birth certificates.
  • Verification of all gross income received by both veteran and spouse on a monthly basis. (Ex.- Social Security, pension, investments, etc.)
  • Marriage license for current spouse and ALL previous marriage divorce decrees or death certificates for veteran and spouse.
  • Complete list of Assets for veteran and spouse; provide current statements of all account with the veteran or spouse’s name on them.
  • Last 6 months of medical expenses for veteran and spouse. (Ex.- Nursing Home Bill, Assisted Living Bills, Home Health Care Bills, Pharmacy receipts, etc.)
  • Death Certificate of Veteran or most recent Spouse.
  • Voided Check for direct deposit of benefit.
  • Power of Attorney, guardianship or conservatorship papers.
  • Are there any dependent children at home?        YES / NO
  • Has the veteran ever applied for Medicaid?        YES / NO
  • Does the veteran or spouse live in Nursing Home or Assisted Living?    YES / NO
  • Did the surviving spouse live continuously with Veteran?   YES / NO
  • *Please provide all documents listed above as quickly as possible. Applications may be rejected by the VA if all documents requested are not attached to application.
